You must submit this form to confirm your participation in HWP's Sponsor-A-Family program by November 20th. These instructions will also be available in your confirmation email.

How much should I spend on sponsoring the family I've been assigned?

We recommend including the following in your Sponsor-A-Family package:

• A grocery store gift card in the amount of $150
• For each family member (mother or child), we suggest spending approximately $100 per person. This can include a mix of gift cards from our Gift Card Suggestion List in any denomination.
• Please include a pair of new pyjamas for each member of the family, in their preferred size. Details on sizes and preferences will be provided once your family has been assigned.

How do I prepare the package for dropping off?

• Please place the gift cards for each family in separate sealed envelopes, clearly labeled with the corresponding family number.
• Put this envelope together with the PJ sets in a sealed bag, labeled clearly with the family number.
• Include your full name, address, and assigned family number on the packages and the gift card envelopes.
• Clearly write the value on each gift card and attach the activation receipt ONLY to each individual gift card.
• Include original purchase receipts for the PJ sets.

When and where do I drop off my package?

Gift Card Drop-Off Dates: December 3 to 8

Milton and Burlington Shelters
Thursday, December 3 to Friday, December 4 | 10:00 a.m. to 6:00 p.m.
Monday, December 7 to Tuesday, December 8 | 10:00 a.m. to 6:00 p.m.

Burlington Shelter only
Saturday, December 5 | 9:00 a.m. to 12:00 p.m.

No drop-offs on Sunday, December 6.

Please note: HWP will not issue charitable tax receipts for gift card donations or pyjamas without the original purchase receipts included with your donation.
